Our Disability Support Coordination team are all local to the area, they know their local regions and who are highly skilled at sourcing a diverse range of supports from specialist disability to therapeutic services and mainstream supports. They will support you to choose how you want your services to be provided and who you want to provide them. And then they can help you find the right providers, and get those services set up – including supporting you to decide the budget for each type of support and negotiating prices and service agreements.

There are three levels of support coordination that can be included in your plan:

- Support connection – builds your ability to connect with informal, community and funded supports so that you can get the most out of your plan and pursue your goals.

Coordination of supports – builds the skills you need to understand and use your plan. A support coordinator works with you to make sure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be part of your community.

Specialist support coordination – a higher level of support coordination for people with more complex needs and situations who need specialist support. A specialist Support Coordinator will assist you to manage changes and help ensure consistency.
